One of our objectives is to make high-quality oral healthcare as convenient, accessible, and affordable as possible for our patients. To do so, we will work with you directly to maximize reimbursements from your dental insurance provider. And before your procedure, we will provide you with the most accurate payment estimates we can obtain from your insurance carrier.
Insurance Information
Please remember that your insurance provider only conveys estimated benefits to us and does not guarantee payment for services.
Typically, insurers do not cover all dental expenses. Dental insurance is designed to reduce your out-of-pocket expenses, but there will still be some out-of-pocket expenses you need to pay at the time of service.
Also, be aware that our practice does not participate with Medical Insurances, Medicare, Medicare replacements, or Medicaid.

As a courtesy to our patients, we will file claims on your behalf to your insurance company. We will file primary and secondary dental claims only. Any additional filing will be the responsibility of the insured. For patients with out-of-network insurance, we require payment in full, and the insurance company will reimburse you for any covered service. If your insurance will not provide us a copy of the Explanation of Benefits for the claim, you must provide us a copy. If we are not provided the EOB, we will not be able to submit a claim to any secondary insurance carriers. Our office is “opted out” for Medicare; this means we are unable to submit a claim to Medicare and the patient is unable to submit a claim to Medicare, including supplemental Medicare insurance. Please make sure to read, sign, print, and date the Medicare Opt Out Private Contract form. Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance or any other balance not paid for by your insurance company. You will be responsible for all collection costs, attorney’s fees, and court costs.
Financial Information
While we will do our best to maximize your dental insurance benefits, the patient or legal guardian is ultimately responsible for the cost of treatment if the insurance company does not pay.
