During your initial appointment, Dr. Fisher will discuss the diagnosis and treatment options and offer plenty of opportunities for you to ask questions. While they may be able to provide treatment on the same day as the consultation, it is often necessary to return for surgery on a later day.
One way you can ensure a smooth consultation is to bring the following items in with you to your first visit:
- A valid driver’s license or photo ID
- Your dental and medical insurance card(s)
- The referral form and/or any x-rays given to you by your referring doctor (if applicable)
- A current list of medications that you are presently taking (heart medications, aspirin, anticoagulant therapy, etc.)
- Form of payment (credit card, checkbook, cash, etc.)
Additionally, let us know in advance of your appointment if you have a medical condition that may be of concern prior to surgery. Common examples include diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.
IMPORTANT: A parent or guardian must accompany patients under the age of 18 during the consultation.
